This program strongly supports one’s willingness to live a healthy life and manage diabetes.
ABOUT OUR PROGRAM
Nothing new! This program is population-focused, intentionally designed to reduce risk or promote health before the onset of diabetes, as well as to introduce practical and workable management of diabetes to improve health and quality of life targeted to specific levels of risk.
We are mainly re-doing steps, re-setting habits, re-visiting what’s difficult, and re-defining how to manage diabetes based on a motivation to change! That’s all, really!
We do things relatively “hands-on” with additional adaptive methods and techniques that accommodate those who are already experiencing vision loss due to diabetes through Vision Rehabilitation Teaching.
There is a high prevalence of individuals living in New Zealand with diabetes and with diabetes comes an increased risk of vision impairment/blindness. Individuals may not have had adequate opportunities to educate themselves about nutrition and healthy eating, which are essential for effectively managing their condition.
With this initiative, we aim to offer practical knowledge based on vision-related adaptive skills through personalised group instructions and hands-on learning experiences on how to manage life with diabetes, and live safely and well with vision loss.
This Rehab Training Program provides an integrative approach to learning through experience. Our learning is based on our concept of the 4-Ms:
MONITOR - for people impacted by diabetes starting with learning adaptive methods on how to monitor glucose levels using assistive technology to guide and record daily results;
MEDICINE MANAGEMENT- further education and best practices in maintaining timely administration of medicines;
MEAL PLAN - hands-on activities in preparing healthy meals that are simple, accessible, and easy to adapt portion/sizing techniques, including adapting proper way in calculating calorie-intake on a daily basis;
MOVEMENT - to implement and encourage low-impact exercises done at home together with the family/whanau -- which in general will facilitate positive lifestyle changes that align with the goal of effective diabetes management, and mainly may prevent further vision loss and/or blindness.
