  • Begin your journey by being referred to our program through your healthcare provider, community agency, or self-referral. 

  • A rehabilitation specialist will contact you via phone to discuss your needs, answer questions, and schedule a comprehensive assessment.

  • In-depth evaluation of your vision, functional abilities, and daily living needs to tailor your rehabilitation plan.

  • Receive personalized training sessions based on your assessment, including: 

    • Individualized rehabilitation exercises 

    • Adaptive equipment recommendations and training 

    • Support for daily living skills to enhance independence 
      Training can be delivered at home, in the community, or at our facilities, depending on your preference and needs. 

  • Regular check-ins to review your progress, adjust your rehabilitation plan, and address any challenges.

  • A final session, conducted either by phone or in person, to summarize your achievements, discuss ongoing strategies, and plan next steps.

  • We’ll reach out to ensure your continued success, address any new concerns, and provide additional support if necessary. 

  • If new or ongoing needs arise, we facilitate re-referral to continue your rehabilitation journey or access other services.
